What are the responsibilities and job description for the Social Worker - LMSW - Hospice Care position at True Hearts and Hands Hospice, LLC?
True Hearts and Hands Hospice is searching for an LMSW to join our team to provide Social Work services for our patients and their families. Position can be PRN to Part-Time.
Medical Social Workers:
Assess hospice patients to identify psychosocial, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals
- Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
- Meet mandatory continuing education requirements of the agency/licensing board
- Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
- Promote Agency philosophy and administrative policies to ensure quality of care.
- Attend interdisciplinary team meeting, participate in the patient care planning process and collaborates with Interdisciplinary Team to promote coordination of patient care
- Provide superior patient care, and support to patients family
- Utilizing all tools available, effectively communicates patient/family needs and updates to Interdisciplinary Team.
- Participate in the training of care staff.
- Participate as a integral part of the True Hearts and Hands Hospice Team.
Position Requirements Include: Current Texas State license as a Medical Social Worker One year hospice experience required, 3 years preferred
- Ability to work in a field setting and proven ability to make sound nursing judgments
- Professional communication skills
- Basic computer - Microsoft office - skills.
