About the Role
Tk Health is seeking a dedicated mental health professional to join our team in providing patient care where the need is highest. Our correctional healthcare provider offers a unique opportunity to utilize your medical skills in a safe, secure environment.
As a licensed therapist in this correctional setting, you will assess, evaluate and coordinate mental health care for incarcerated individuals, formulating individualized release plans to coordinate a continuum of care.
- You will assess psychiatric emergencies and provide crisis intervention to emotionally distressed and mentally ill inmates.
- Conduct evaluations of inmates for the purpose of suicide intervention and prevention.
- Coordinate inmate care with Facility, and Medical staff to ensure inmates' special needs are addressed.
- MAT/substance abuse counseling will be provided as part of your duties.
- You will develop Individualized Treatment Plans for psychiatric inmates who require intensive behavior management; completes referrals to Provider for medication management services.
- Promote linkages with community mental health agencies, other support services and families for inmate care and coordination for continuum of care; including discharge planning; collaborates with community health providers for inmates who are enrolled clients.
- Complete inmate need assessments and develops individualized release plans for inmates suffering from mental illness to connect inmates with appropriate services in the community.
