Program Nurse

Under the supervision of the Lead Nurse and Program Director, the nurse coordinates the delivery of health care services and participates in direct care. The Nurse shall be on-site 8 hours per day and any additional hours necessary to meet requirements for the provision of health and personal care services. Requirements: Graduate of a Nursing Program approved by the Massachusetts Board of Nursing, current license as a Nurse in Massachusetts (Registered Nurse or License Practical Nurse) Primary Duties: •Participates in preadmission screening of new adults •Communicate with healthcare professionals involved in the individual’s care regarding the provision of the individual’s healthcare. •Evaluate individual health status and report changes to the appropriate responsible medical care provider and/or caregiver. •Maintain individual records and documentation: including physicals, immunizations, seizure protocols, medication orders, ect in accordance with the Department of Public Health, Department of Disability Services, MAP Program, Board of Nursing. •Maintaining up to date Health Packet database •Understanding of MAP Program policies and procedures •Triage and treat individuals with acute health concerns. •Consult with individuals, caregivers and Adult Services Coordinator on concerning acute and ongoing medical concerns. •Communicate with staff regarding acute and ongoing medical concerns. •Assist with service coordination efforts that maintain and/or improve individual health, quality of life, and/or functioning. •Assessment and training of individuals who self-medicate and those who need medication during the day (short term and long term). •Training staff on new health procedures and medical needs of the individuals

Salary : $30 - $42
