Duties include but are not limited to:
- Develop and/or support the scheduling for direct care staff to ensure supports are available to persons served.
- Responsible for coordinating home and community-based services to eligible participants by identifying their strengths and needs and incorporating formal and informal supports through a participant-centered approach to service planning and delivery.
- Responsible for completion of a comprehensive assessment to identify client strengths and needs.
- Communicate staff schedule needs/changes in a timely manner and arrange for replacements.
- Support staff in the implementation of behavioral plans.
- Ensure timely documentation of client interactions and support interventions in the form of progress notes, incident reports, communication log, and other as needed and instructed by direct support staff.
- Make sure that quality services are provided to all participant
- Process Referrals for intake on Therap as admin.
- Daily Referral check on therap
- Coordinate & set up meet and greets
- Coordinate clients INTAKE meeting
- Involve client in the community
- Ability to create Programs for individual on Therap.
- Ability to create required assessments, services and safety plan for individual served.
- Coordinate with the individual's team to develop services, medical need and behavioral support protocols.
- Monitor to ensure that significant changes in the individual's need are addressed immediately, that the service plan is revised, and that gaps in services are addressed by the agency.
- Maintain Individual and Agency record.
- Communicate with agency regarding individual's health concern and take appropriate action.
- Ensure that Programs are run and in compliance with Federal and state Regulation.
- Day-to-Day Operations:
- Ensures staff-to-participant ratios are maintained.
- Morning Announcement to staff and clients
- Ensures all participants are on outings by 9 AM.
- Coaches staff on interactions and addresses participant concerns.
- Morning , lunch time , and afternoon floor walks . ( You should be
- engaging with participants and DSPs) .This will be the perfect
- moment to see how DSPs are doing with the clients .
- Ensure the day site is clean before going home ( include pushing in
- chairs, turning off lights and TVs, ensuring everyone has left, and
- cleaning up dishes left in the sink. Day site vehicles being clean, and clear.
- Communicate with the Director and other coordinators when being late or absent .
- Staff Management & Disciplinary Actions:
- Coaches staff as needed.
- Issues disciplinary action when required, particularly after program audits.
- Program Audits: Conducts daily audits of program data.
- Vehicle Oversight: Ensures vehicle logs are completed and that vehicles remain clean and in good condition.
- Outing Spot Checks: Conducts at least two monthly outing checks to ensure staff engagement, attentiveness, and adherence to ratios is followed
REQUIREMENT / QUALIFICATION:
- Previous Experience with day site leadership HIGHLY preferred
- Experience in creating / writing Program and Assessment on Therap.
- Experience working with individuals with Developmental disabilities.
- Experience in word, excel and outlook.
- Familiar with Therap.
- Knowledge of current practice in the field of community-based services
- Have a valid Driver License
- Pass APS/CPS and Criminal background check.
- Supervise the Area of Medication Management
- Oversee the execution of medication Management.
TRAINING / EXPERIENCE:
- Previous Experience with day site leadership HIGHLY preferred.
- CPR/FIRST Aid certificate.
