What are the responsibilities and job description for the Community Resources Development Coordinator position at Together We Shine?
Primary Responsibilities:
- Lead, recruit, and manage a team of job coaches, overseeing their schedules and stepping in when necessary.
- Ensure job coaches meet vocational rehabilitation documentation requirements.
- Perform comprehensive assessments to determine support needs and create individualized goals for self-sufficiency.
- Collaborate with team members to provide cohesive support to participants.
- Develop and maintain person-centered plans tailored to each participant’s unique needs and objectives.
- Connect participants with relevant community resources and guide them in accessing essential services.
- Monitor and address any barriers to employment and independence, ensuring participant progress and well-being.
Additional Responsibilities:
- Cultivate and sustain relationships with community partners, participants, and stakeholders.
- Keep up to date on available community resources related to healthcare, housing, education, and employment.
- Track participant progress and lead regular team meetings to discuss needs and strategies.
- Identify and facilitate support for independent living, healthcare, mental health services, and more.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, or Human Services required.
- Strong understanding of public benefits and proficient in computer skills and technology.
- Excellent communication, organizational, and relationship-building skills.
- Driver's license and full coverage car insurance.
Job Type: Full-time
Pay: $35,000.00 - $55,000.00 per year
Schedule:
- 8 hour shift
- Monday to Friday
