Basic Function
- Responsible for providing, managing and coordinating all direct service activities for a caseload of 16-19 adults.
- Reports to the Clinical Supervisor
- In conjunction with the Clinical Supervisor and intake staff, determines and identifies the service needs of residents in order to provide services that link clients with permanent housing, health providers’ services, entitlements, vocational services, and family support systems to assist in the maintenance and quality of life of the.
- Assists in identifying new, untapped community resources to increase service utilization to as to meet the diverse needs of residents and assist in maintaining the social, medical, spiritual, and psychological well-being of the clients.
- Documents all direct service activities including clients served, group participation, case closures, direct service/advocacy interventions and referrals.
- As deemed appropriate, participate in volunteer/staff training activities and other educational programs in-house or contracted from the community.
- Participate in ongoing evaluation processes to assess direct service activities for program modification, permanency planning, and to assist in the preparation of required reports.
- Weekly meetings with all residents.
- Weekly contact with HRM/Ryan White Part A Housing Specialists.
- Visits to agencies when needed to facilitate coordination of services and accomplishment of program objectives.
- Escort residents to housing interviews and/or medical facilities when necessary; and conduct client follow-up for clients who have successfully moved into permanent (independent) housing.
- Performs services consistent with the needs of the agency.
- Bachelor’s Degree or Associate Degree with two years of case management experience.
- Prior experience preferred in case management, counseling with groups and/or individuals, crisis intervention and client advocacy.
- Knowledge of HIV/AIDS-related information, substance use and mental health issues.
