Supportive Housing Coordinator
The Supportive Housing Coordinator is responsible for providing case management services to an assigned group of residents living in permanent supportive housing. This position provides strength-based services to support resident goals and program plans focused on housing stability and building self-sufficiency skills. Responsibilities include reporting outcomes and daily notes, responding to crisis, working closely with residents to create goal plans, and supporting action steps to move identified goals forward.
Min. Req.:
- High school diploma or equivalent.
- A minimum of two years case management or crisis management related work
- Meet TCAction criteria for satisfactory driver’s license in order to access agency owned vehicles for program related activities.
