Job Overview:
Top Care Support is Currently seeking a dedicated and experienced Program Manager for our In-Home Supports program to join our team and oversee the delivery of exceptional support services to individuals with intellectual and developmental disabilities (I/DD). In this role, you will have the opportunity to lead a team of Direct Support Professionals (DSPs), ensuring the implementation of personalized, high-quality care plans tailored to meet the unique needs of each individual. Your responsibilities will include program oversight, collaboration with clinical care teams, maintaining positive relationships with individuals' guardians and families, as well as ensuring regulatory compliance and continuous staff training and development. We are looking for a motivated individual with excellent communication skills, managerial experience in the field of I/DD, and a genuine understanding of the diverse needs and abilities of individuals with intellectual and developmental disabilities.
Responsibilities:
- Oversee in-home 1:1 program activities, manage a team of Direct Support Professionals (DSPs), ensure individual goals are achieved, customize Individualized Support.
- Communicate and collaborate with internal and External Clinical Care Teams to address individuals clinical needs
- Lead a team of DSPs, ensuring they receive adequate training and supervision, and are matched appropriately with clients.
- Maintain positive relationships with the Individuals guardians, family, and Support Coordinator.
- Regularly evaluate the performance of Direct Care staff to ensure delivery of Quality services.
- Oversee scheduling for assigned Individuals and ensure coverage when necessary.
- Conduct New Hire Orientation and individualized specialty training as needed.
- Perform intake meetings with new individuals.
- Ensure the program complies with relevant regulations and standards.
- Work with internal teams, such as Human Resources, Compliance, Administrative, and Department Directors.
- Attend meetings as required, including ISP, Intake meetings, and others as necessary and file all paperwork properly as required.
- Stay informed about current research, best practices, and emerging trends in the field of intellectual and developmental disabilities. Continuously educate yourself and others to promote greater understanding and acceptance.
Qualifications:
- Excellent verbal and written communication skills
- Experience working with individuals with Intellectual and Developmental Disabilities in a managerial role.
- High school diploma / bachelor’s degree
- Ability to work independently without constant supervision.
- Must possess positive, enthusiastic attitude, and good people skills.
- Possess a keen understanding of the unique needs of individuals with I/DD
- Experience or training in crisis management and intervention.
- Strong Problem-Solving and Conflict Resolution Skills
