Job Summary: The Case Manager will play a crucial role in coordinating and providing support for clients with substance use disorders. This position involves assessing client needs, developing and monitoring individualized treatment plans, and connecting clients with appropriate resources. The Case Manager works closely with a multidisciplinary team to ensure that clients receive the care and support they need for recovery and well-being.
Key Responsibilities:
- Assess Client Needs: Conduct intake assessments and interviews to gather comprehensive information about the client's substance use history, and personal circumstances.
- Develop and Implement Treatment Plans: Work collaboratively with clients and the treatment team to create individualized treatment plans tailored to each client’s needs, goals, and progress.
- Monitor Client Progress: Regularly follow up with clients to evaluate their progress toward treatment goals, adjusting plans as needed and addressing any challenges.
- Provide Support and Advocacy: Act as the primary point of contact for clients, offering emotional support and advocacy to ensure they understand and follow their treatment plans.
- Coordinate Services: Link clients to community resources and external services such as housing, medical care, social services, legal support, and vocational training.
- Document and Record Keeping: Maintain accurate, up-to-date documentation of client interactions, progress, and any changes in treatment plans or needs, ensuring compliance with facility policies and HIPAA regulations.
- Crisis Intervention: Respond to crisis situations, providing immediate support and guidance, and collaborating with clinical staff and emergency services when necessary.
- Collaborate with Team: Work with a multidisciplinary team, including therapists, physicians, and other behavioral health professionals, to coordinate comprehensive care and ensure holistic treatment.
- Educate Clients: Provide education on substance use disorders, recovery strategies, and coping mechanisms to empower clients in their recovery journey.
- Ensure Compliance: Ensure clients are following treatment protocols, attending necessary appointments, and adhering to the facility’s policies and guidelines.
- Case Documentation and Reporting: Maintain timely and accurate documentation for case management activities, client progress, and treatment goals to ensure the effectiveness of interventions and compliance with regulations.
Qualifications:
- Bachelor’s degree in social work, psychology, counseling, or a related field required
- Previous experience in a case management role in a mental health, substance abuse, or social services setting is highly preferred.
- Knowledge of mental health and substance use disorders, including common treatments and recovery strategies.
- Strong understanding of community resources and social services available for clients in need.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work effectively with individuals from diverse backgrounds and those in recovery.
- Compassionate and empathetic approach to client care.
- Ability to handle crisis situations with patience and professionalism.
- Must be able to work both independently and as part of a team.
- CPR and First Aid certification preferred (or willingness to obtain).
