About the Role

Responsibilities
The successful candidate will be responsible for:
- Conducting home visits to assess client needs and develop care plans
- Administering medications, wound care, treatments, and procedures as prescribed by physicians
- Monitoring client conditions and reporting changes to clinical or client services managers
- Following up with physicians to ensure accurate documentation of orders
- Performing client assessments as necessary to identify areas for improvement
- Casework management and coordination to ensure seamless client care
- Accurately documenting observations, interventions, and evaluations related to client care management and services provided
Requirements
- Current RN licensure in the state of practice
- Minimum 1 year of experience in home health care or a related field
- Excellent communication and interpersonal skills
- Ability to work independently with minimal supervision
- Strong organizational and time management skills
