POSITION: RNCM
REPORTS TO: Clinical Supervisor
POSITION SUMMARY
Provides direct care assessments and admissions' F2F for Agency referral clients in accordance with company policies. Patient care skilled nursing. Strong documentation skills.
QUALIFICATIONS
- Graduate of an accredited school of professional nursing.
- Current license as a Registered Nurse in the state(s) of practice. Baccalaureate degree preferred.
- Minimum of two (2) year of nursing experience. Home care experience preferred.
- Demonstrated strong and effective written and verbal communication skills and good interpersonal skills.
- Ability to deal tactfully with clients and the community.
- Demonstrated organizational and time management skills.
- Knowledge of home health regulatory and reimbursement requirements.
- Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair.
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
1. Reviews client referral information and responds to requests and inquiries as appropriate.
2. Performs initial and possible ongoing client assessments based on Agency policy and standards of practice to ensure effective and appropriate home care services. a. Performs comprehensive assessments including OASIS data collection where indicated.
3. Directs, plans, and initiates appropriate action independently and responsively in home care situations.
4. Collaborates with supervisors to identify staffing needs and evaluate qualifications and competencies of current and new employees.
5. Secures written confidential documents in a manner that prevents unauthorized release.
6. Provides skilled nursing visits based on adult and pediatric client assessed needs. (Infusions, labs, wound care, medication set up)>

Pay: $40.00 - $45.00 per hour
