To provide assistance and understanding to the family in the home care situation and in time of bereavement. To work as a member of the Hospice team in providing Hospice care. JOB SUMMARY : A qualified Licensed Practical / Vocational Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies. JOB SUMMARY The Scheduling Coordinator is responsible for coordinating patient care services by assigning nursing and therapy staff, managing visit schedules, processing orders, and providing operational support to field staff. This role ensures compliance with agency standards, regulations, and patient care requirements to facilitate efficient and effective service delivery.
RESPONSIBILITIES
- Assign nursing and therapy staff to newly admitted patients.
- Schedule patient visits in MyUnity according to orders from admitting staff.
- Monitor staff compliance with scheduled visits; document missed visits as needed.
- Schedule OASIS visits within the 5-day window at the end of each certification period.
- Routinely check and process physician orders from field staff.
- Handle drop-ship requests and coordinate medical supply orders as required.
- Provide support to field staff through phone, email, or in-person assistance, including managing schedule adjustments and time-off requests.
- Notify staff of any changes in patient status, such as hospitalizations or insurance holds.
- Fax medication interactions to the patient’s primary physician and notify the Team Coordinator.
- Serve as backup to the receptionist, answering all incoming calls by the third ring.
- Multitask and adapt to changing priorities and responsibilities throughout the day.
- Perform additional duties as assigned to support agency operations.
- Must be available for occasional travel and weekend work.

Requirements : QUALIFICATIONS
WORKING ENVIRONMENT
Primarily an office environment with a moderate noise level, involving regular interaction with personnel, patients, and visitors.
JOB RELATIONSHIPS
Supervised by : Director of Clinical Services / Clinical Manager
Works with : Nursing staff, therapy staff, field staff, receptionist, and other administrative personnel.
RISK EXPOSURE
The role is considered to have low to moderate risk exposure, primarily involving an office environment with limited direct patient contact. However, occasional exposure to stressful situations and handling of sensitive patient information requires adherence to confidentiality and agency protocols.
LIFTING REQUIREMENTS
The role requires the ability to sit, talk, and hear regularly, with frequent hand use for handling or reaching. Occasional lifting or moving of items up to 25 pounds may be necessary. The ability to perform tasks while sitting or standing for extended periods is essential to fulfilling daily responsibilities.
