Housing Case Manager - Hennepin County
Pay Range: $47,000 to $50,000 annual salary, based on qualifications
Benefits: Full time, benefit eligible
Schedule: 5 Day Workweek (8 hours per day) or 4 Day Workweek (10 hours per day)
Location: Bloomington/ Minneapolis
The Housing Case Manager provides Housing Support Supplemental Services (HSSS) and Integrated Community Services (ICS) to residents living in an apartment complex in Bloomington along with some clients living in Minneapolis.
Housing Case Manager Duties:
- Regularly meet with clients in their apartments (Bloomington), in their homes (Minneapolis) and other locations as needed throughout the community.
- Establish supportive and motivational relationships with clients while providing HSSS and ICS services.
- Complete onboarding process for new clients, including assessments needed for HSSS and ICS services.
- Connect clients to community resources and programs.
- Maintain client records in electronic health record system.
- Collaborate with other family members, service providers, staff members, consultants, interns and/or volunteers.
Housing Case Manager Requirements:
- Experience providing Housing Support Supplemental Services is highly desired
- Experience providing Integrated Community Services is highly desired
- Experience working with individuals who have experienced long-term homelessness is desired
- Experience working with individuals affected by mental health and/or chemical dependency issues is desired desired
- Bachelor's degree in a behavioral health field preferred
- Valid driver's license, vehicle insurance, and acceptable driving record required
