Job Specific Essential Duties
- Perform case management duties including timely completion of required documentation and maintenance of confidential client files. Ensure TMI database is accurate and current.
- Participate as a member of the interdisciplinary/planning team.
- Meet regulatory meeting compliance and provide support services in accordance with client level of care and funding requirements. Attend additional meetings as necessary (IPP, medical, psychiatric, school, etc.).
- Develop and implement all required documentation to include quarterly reports, incident reports, and continuation of service reports according to established deadlines.
- Submit Individualized Family Support Plans (30-Day, Q4, and annual) to program director for review by the 10th of the following month in which the meeting is required.
- Develop individualized adaptive support materials as needed (visual schedules, reward systems, social stories, etc.)
- Participate in the development of a circle of support, both paid and unpaid, to build relationships with family, friends, neighbors, and other community members.
- Develop and expand client’s participation in integrated community activities.
- Support client’s choices and interests by identifying and implementing preferred activities.
- Serve as a liaison to client’s family, the Regional Center, and other public service agencies.
- Coordinate and consult with staff, placement agency, funding sources, schools, and families to ensure effective implementation of recommended program and services.
- Provide program training and guidance to Resource Parents in efforts to comply with licensing and approval requirements.
- Provide regular monitoring of regulations requirements including but not limited to: P&I Logs, Medication Logs, client confidential records, parent records, fire drills, and general safety inspections. Review and approve additional documentation on a quarterly, semi-annual, and annual basis.
- Complete Resource Family home semi and annual checklist (Title 17).
- Participate in scheduled clinical meetings (2) per month.
- Participate in Family Support Services sponsored events.
- Participate in New Resource Parent Orientation & three-part training, every quarter.
- Complete PRO-ACT training to obtain and maintain up to date TMI PRO-ACT certification.
- Along with other daily general essential job duties
Minimum Experience and Qualifications
- Education: Master's degree in human services or related field
- Experience: 2 years working in social services or with people with disabilities
- Driving: 3 years driving experience;
