Job Title: Administrator & Clinical Supervisor                              Hours: (35) per week
 
Union Affiliation: Middle Management Union                              Salary Range: $112,477 - $134,041/annually
 
POSITION OBJECTIVE:
Serves as the chief professional officer, accountable and responsible to the Board of Directors (BOD).  Responsibilities include the delivery and administration of comprehensive home health services to the individuals, family and community of Berlin and its surrounding towns.  Provides direct supervision of all personnel in the Department of Nursing. Ensures clinical services are provided in compliance with state licensure regulations, Medicare and Medicaid conditions of participation and Joint Commission Standards, and in accordance with the Nurse Practice Act. 
 
DUTIES/RESPONSIBILITIES:
  • Plans, directs and supervises the daily operations of homecare operation.  Develops objectives, standards of care, policies and procedures affecting clinical services.  Administers and/or oversees staff orientation, in-service education, program planning. Evaluates clinical competence and case management ability of nursing personnel. Prepares a written plan for the delegation of administrative responsibilities and authority in event of the absence of the Director.
  • In conjunction with the Director of Pupil Personnel Services and the School Nurse Coordinator, oversees a comprehensive School Health Program. 
  • Attends all Board and committee meetings as an ex-officio member without vote but with the right of discussion.  Provides guidance to the BOD and committees regarding trends and issues in home health care and community nursing services and gives leadership and direction in setting goals. Serves as the custodian of all documents and records of the VNA.  Plans, budgets and evaluates programs based upon community needs and resources.  Is actively involved with the BOD in the process of strategic planning for the VNA.
  • Assures an adequate public information system in consultation with the BOD.  Represents the VNA to other groups, organizations and the general public.
  • Maintains and develops reports in compliance with regulations.  Ensures that all programs are in compliance with federal, state and local regulations and statutes. Addresses public and professional inquiries. Ensures that information systems are adequate and accurate.
  • Develops and maintains a record system and statistical reporting system of program documentation, planning and evaluation, which includes at least the data specified by the Federal Medicare Conditions of Participation, the State of Connecticut Licensure regulations and the Community Health Accreditation Program (CHAP) standards. Maintains agency compliance with Federal, State and CHAP Licensure regulations and standards.  Directs and monitors organizational Performance Improvement Activities.  Develops and implements standards of service for VNA programs.
  • Prepares a budget in conjunction with Finance Committee for the approval of the governing authority and implements financial policies, accounting system and cost controls
  • Directs nursing and home healthcare as prescribed by the attending physician.  Reviews all plans of treatment for accuracy, appropriateness and completeness.  Assures reports to the physician are made regarding the patient’s response to treatments, including drugs and changes in the patient’s physical or emotional condition.
  • Evaluates and regularly reevaluates the therapy and nursing needs of each patient.  Ensures all aspects of patient care are provided in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Provides wound care instruction and evaluation for patient care.  Assures that case conferences are done on each patient, and that they are recorded and filed in patient’s chart.
  • Develops methods for recording documentation of all services rendered by the Agency and implements any changes in documentation procedures as necessary. Works with Clinical Record Review, Professional Advisory and Quality Assurance Committees of the Agency.
  • Stays current on new methods, products, trends, etc., and communicates this information to clinical staff.  Develops an annual competency program.  Participates in the agency’s information management and performance improvement programs; collects, analyzes and graphs information.
  • Performs other duties as assigned.
 
QUALIFICATIONS:  An Agency Supervisor of Clinical Services / Clinical Manager shall be a registered nurse with an active license to practice nursing in this state, and shall have one of the following:
  • A Master’s Degree from a program approved by the National League for Nursing or the American Public Health Association with a minimum of one (1) year full time clinical experience in a home health agency or relating community health program which included care of the sick at home; or
  • A Baccalaureate Degree in Nursing and a minimum of three (3) years of full-time clinical experience in nursing, at least one (1) of which was in a Home Health Agency or Community Health Program which included care of the sick at home; or
  • A Registered Nurse who has been continuously employed in the position of Supervisor or Clinical Services in a Home Health Agency in this state since January 1, 1979; or
  • A Diploma in Nursing or an Associate’s Degree in Nursing and
  • A minimum of three (3) years of full time or full time equivalent clinical experience in nursing within the past five (5) years, at least one (1) year of which was in a Home Health Care Agency or Community Health Program which included care of the sick at home; and
  • Evidence of certification by the American Nurses’ Association as a Community Health Nurse or completion of at least six (6) credits received within two (2) years in Community Health Nursing theory or six (6) credits in Health Care Management from an accredited college or university program or school of nursing.
 
SKILLS:  Comprehensive knowledge of general nursing theory and practice.  Demonstrated ability in supervision and leadership. Ability to coordinate patient care services. Wound care certification highly desirable. Must be able to communicate verbally and in writing with staff, administrators and the public.  The ability to carry on the physical and mental functions for a staff nurse and must be free from addictive habits.
 
JOB ENVIRONMENT:  Work is generally performed under typical office conditions.  The work involves moderate risk or discomfort in requiring on-site home supervision at various intervals throughout the year.  The employee may be required to use protective clothing or equipment such as masks, coats, boots, goggles, gloves or shield.  Operates computer and standard office equipment.  Must have a valid driver’s license.  Makes frequent contact requiring perceptiveness and discretion with other Town officials, physicians and hospitals or other health institutions, local, state or federal agencies and the public or other individuals as deemed necessary.  Error in decision could result in lower standards of service with danger to public safety and errors in supervisory decisions could result in excessive costs for both patients and legally, the Town.
