JOB SUMMARY

The Mental Health Rehab Specialist/Case Manager III reports to the Program Director and is responsible for providing specialized services to clients in gaining access to needed medical, social, housing, economic, educational, and other direct services.  

ESSENTIAL JOB RESPONSIBILITIES

  1. Assist in the accomplishment of the treatment plan for Children's Mental Health program clients under the supervision of the Program Director or Clinical Supervisor, by screening, evaluation and identification of specific case management activities.
  2. Employees shall follow the reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse and Tarasoff warnings per California reporting laws. 
  3. Will manage referrals via telephone, walk-in, fax or email.
  4. Ensure all new referrals are logged and entered into the Electronic Medical Record (EMR) in a timely manner.
  5. Meet directly with parents/guardians to complete initial admission documentation and enter new episodes in in the EMR.
  6. Will provide crisis evaluation assessments, including the use of de-escalation techniques, develop contingency plans, make recommendations for involuntary hold placement and interface with county psychiatric emergency team staff. Will enter new episodes in the EMR as part of crisis documentation.
  7. Will provide therapeutic behavioral services (TBS) to consumers, as assigned by Team Leader.
  8. Will provide personal service agreement (PSA) services, as assigned by Team Leader.
  9. Will provide mental health related rehabilitation services to identified and assigned clients.
  10. Will help client’s learn new and improve upon their social skills.
  11. Will maintain a concrete behavioral focus while working with clients.                                                                                                                                                  
  12. Will conduct targeted case management evaluations and will refer cases to appropriate agencies as indicated.
  13. Will document all progress notes in the client’s Electronic Health Record (EHR) by 10:00 am of the following business day.
  14. Will attend job-related trainings and also participate in a variety of in-service training, seminars, and practicum.
  15. Will attend weekly staff meetings and weekly supervision meetings.
  16. Perform other tasks when asked by the Team Leader or Program Director, as time allows.
  17. Will drive on Agency business.
  18. Physical presence at the program is required.
  19. Will think and act quickly and efficiently in emergencies.

ESSENTIAL JOB REQUIREMENTS

  1. Bachelor’s degree in social work or counseling in a related field and four (4) years of full-time work experience providing direct client services in a mental health, social service, educational or similar environment.
  2. Ability to communicate effectively in written and spoken English.
  3. Work effectively with diverse ethnic, religious, socioeconomic and professional groups of people.
  4. Plan, coordinate, and facilitate services for clients of the Children's Mental Health programs.
  5. Assist the Mental Health Professional and Clinical Psychologist in screening and identification activities.
  6. Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
  7. Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required. 
  8. Ability to pass a criminal background check.
  9. Ability to pass a pre-employment drug screen and TB test.

 ESSENTIAL JOB REQUIREMENTS FOR BILINGUAL SKILL

  1. Communicate effectively in written and spoken Spanish.
  2. Required ability to communicate effectively with monolingual Spanish speaking clients and families for all clinical purposes.
  3. Assist with phone calls for Spanish speaking clients.
  4. Required to provide language translations for case managers and Physicians on a periodic basis.
  5. Minimal adequate competency in the Spanish language as evidenced by successful passage of the TPOCC Spanish Competency Test.
  6. Other translation tasks as required.

    • Monolingual: Class 125 ($21.75 – $26.46/hour)
    • Bilingual: Class 130 ($21.13 – $27.81/hour)

  • Qualified non-profit for the Public Service Loan Forgiveness (PSLF) Program
  • Regular work schedule (Monday through Friday, 8am - 5pm)
