Job Title: Registered Nurse, RN - Hospice Admissions Nurse
Shift: Monday thru Friday 8:30am to 5:00pm
Job Location: Lackawanna County and surrounding areas
Job responsibilities include, but not limited to the following:
- Admits patients to Traditional Home Health Care, completing clinical documentation as required.
- Assesses patient status and develops plan of care with coordination of nursing supervisor, patient and family members, and physician.
- Coordination of hospice referrals- obtaining records / orders / diagnosis
- Initial evaluations for hospice appropriateness
- Obtaining consents/initial hospice discussion with patients/families
- Maintenance of electronic hospice contracts
- Coordination and auditing of crisis care team
- Hospice philosophy teaching
- Coordination of care audits with our SNFs
- Functions as back up to hospice care team when supervisor is out of office
Requirements:
- Current license as a Registered Nurse in Pennsylvania
- BLS Certification
- Must have current driver's license
- Ability to travel to case assignments
- Excellent assessment skills, documentation skills and ability to meet deadlines
- Ability to work independently with minimal supervision and manage time effectively
- Strong interpersonal skills
- Demonstrates good verbal and written communication, and organizational skills
- Acute care and/or home health experience is preferred.
