Case Manager responsibilities include:
- Providing initial and annual psychosocial assessments
- Update the person-centered plan annually or with change in level of need
- Able to engage, screen and use treatment matching protocols
Successful candidates will need:
- Ability to work in a fast-paced environment
- Bachelor's degree in human services field
