Job Summary
Intake/Patient assignment is a very important role any agency.

Below are the list of task Intake is responsible for.
-Verification of Insurance and referral assignment.
-Communicate daily with staff such as social workers, case managers, admission directors, nurses, physicians from hospitals, nursing homes, assisted living facilities, and physicians offices.
-Facilitate new referrals, receiving referrals, and admission contracts to provide appropriate home care services. Responsible for all areas of the admissions process.
-Provided and maintain an accurate account of the facility daily census, reviewed referrals, and insurance verification. -Interact with field staff, scheduling their visits with families and or guardians to where patient care is to be provided.
-Keep in great detail the phone calls and other forms of communications through out the day to provide all other office staff on what is going on with a patient.
Qualifications and Skills
-Experience in Home Health Care Intake setting.
-Preferred background in nursing or health care related degree.
Job Type: Full-time
Pay: $15.00 - $17.50 per hour
Expected hours: 40 per week
