Job Summary
As a Case Manager, you will be responsible for providing support and guidance to individuals coming home from prison. Providing assistance, helping them navigate various social services and resources to improve their overall well-being.
Responsibilities
- Conduct assessments to determine clients' needs and develop individualized service plans
- Coordinate and monitor services to ensure clients' needs are met effectively
- Advocate for clients to access necessary services such as workforce development programs, Section 8 housing, addiction counseling, and behavioral health services
- Utilize motivational interviewing techniques to empower clients towards positive change
- Provide crisis management support when needed
- Maintain accurate and confidential client records
Requirements
- Bachelor's degree in Social Work or related field preferred
- Previous experience in social work or related field is a plus
- Knowledge of community resources and social service programs
- Strong communication and interpersonal skills
- Ability to work effectively with individuals from diverse backgrounds
- Certification in crisis intervention or willingness to obtain certification
Job Type: Full-time
Pay: $23.00 - $27.00 per hour
Expected hours: 40 per week
Schedule:
- 8 hour shift
- Monday to Friday
