What does a Transitions at Home RN Case Manager do?
As an RN Case Manager, you’ll provide skilled nursing care—including assessments, medications, and treatments—while coordinating services and supporting patients and their families in the comfort of their own homes. You’ll work under the direction of the RN Clinical Manager, following physician orders and a personalized plan of care.
Hours:
- Flexible Hours – Monday through Friday, no night shifts! Typically our clinicians work Monday-Friday during the hours of 8:00AM-4:30PM, however we have the flexibility to work with your schedule!

What You Need to Apply:
Active Wisconsin RN License
2 years of nursing experience (1 year of Home Health preferred)
CPR Certification
Valid WI Driver’s License & Car Insurance
2 years of nursing experience (1 year of Home Health preferred)
