As a Call Center Representative for Transitions Care you will be responsible for all incoming telephone calls for a multi-state growing healthcare organization encompassing multiple services lines. Service lines include hospice care, palliate care and primary care.
Major Responsibilities
- Answer all incoming calls in a professional manner, addressing the needs of the caller to the standards of the organization
- Assist staff with requests as needed-scheduling, faxing, phone calls, requests, etc
- Perform data entry of patient information including a coordination note regarding the reason for the call
- Address all incoming faxes including forwarding to the appropriate team member, filing in the patient’s medical record, etc.
- Updating demographic & facility information as requested
- Assist with incoming visitors, packages and guests at the front desks as time permits
- Participate in staff meetings, department meetings, team meetings, briefing, in-services, and other related activities as needed
- Adherence to a code of conduct conducive with Transitions Hospice is expected
- Interact with all staff in a positive and motivational fashion supporting the Company mission
- Conduct all business activities in a professional and ethical manner

Requirements
Education and Experience
- Minimum age requirement of 18
- High School diploma and college degree preferred
- Minimum of 2 years medical office secretarial experience preferred
- MA preferred
- Knowledge of medical technology
- Proficient PC skills (Microsoft Word and Excel preferred)
- Excellent critical thinking skills
- Ability to work in an interdisciplinary team preferred
- Compliance with accepted professional standards and practices
- Satisfactory references from employers and/or professional peers
- Satisfactory criminal background checks
- Self-directed with the ability to work with little supervision
- Flexible and cooperative in fulfilling all obligations
Salary : $21 - $23
