Organization Description: Transitions Clinic Network (TCN) supports a national network of clinics that support individuals with chronic health conditions recently released from incarceration. Founded on the idea that the people closest to the problem are also closest to the solution, each clinic that adopts the TCN program employs a community health worker (CHW) with a history of incarceration as part of the primary care team. Developed in San Francisco in 2006, TCN has supported sites in 14 states and Puerto Rico in implementing the TCN program.
TCN provides technical assistance and training to health systems including providers and CHWs, creates a forum for sharing best practices, promotes quality improvement and supports program evaluation. TCN strives to amplify the voice of those impacted by incarceration and mitigate some of the long-lasting harms of mass incarceration.
This position is primarily remote but will require occasional in-person meetings at our UCSF offices at 1855 Folsom St in San Francisco. Candidate must be in located in California (preferably Bay Area region).
Position Summary: TCN seeks a Network Program Assistant (PA) to support communications, TCN administrative and clinical programs. The PA works both independently and collaboratively with the full TCN team of physicians, program staff, evaluators and CHW’s as well as with a range of external stakeholders.
This requires strong organizational skills, ability to juggle multiple tasks and willingness to work with a diverse and fast-paced team.
Primary Duties for this position:
- Provide direct administrative support to TCN’s Executive Director, Associate Director, and program management staff, including scheduling, organizing meetings, notetaking, and assisting with general operational tasks.
- Lead and maintain TCN’s communications efforts, including managing social media platforms (LinkedIn, Instagram), developing outward-facing written and visual content, and overseeing newsletters (national network and quarterly updates). Assess engagement metrics to refine outreach strategies.
- Support planning and logistics for online and in-person training sessions, monthly and quarterly provider and CHW meetings, and TCN’s annual two-day conference.
- Support development and ongoing logistics for TCN trainings including, curriculum development, content management and online training platform.
- Contribute to grant applications, program reports, and other deliverables, ensuring timely and accurate submissions.
- Assist in policy development and advocacy through research and content verification.
- Additional responsibilities, as assigned and time permitting.
Required Qualifications:
- Strong administrative and organizational skills with the ability to manage multiple tasks and deadlines.
- Experience working independently, demonstrating initiative, and problem-solving in a dynamic environment.
- Strong written and verbal communication skills, including experience drafting newsletters, reports, and social media content.
- Experience in using Microsoft Word, PowerPoint (or similar software) and Canva for creation of educational tools, presentations and implementation tools.
- Ability to assist with project management and organization through Microsoft Calendars, email, and Monday.com.
- Experience using Microsoft Excel (or similar software) to track and summarize data.
- Detail-oriented.
- Strong research and critical thinking skills, with the ability to synthesize complex information for diverse audiences.
- Commitment to amplifying the voices of individuals and communities impacted by incarceration and supporting their leadership in systems change.
- Ability to work effectively with a multi-disciplinary team.
Preferred Qualifications:
- Experience supporting administrative functions in a nonprofit or healthcare setting.
- Experience in communications, media relations, and/or public relations for mission-driven organizations including implementing strategies to increase engagement.
- Experience collecting and organizing data.
- Experience with event planning and supporting event logistics.
- Previous experience with grant reporting.
- Personal experience or experience working with individuals and/or communities impacted by mass incarceration.
- Interest in social determinants of health and the intersection of race, class, and gender in healthcare systems.

Travel: Position may require some travel (~1 day per month).
Position requires the ability to lift 25-pound boxes to a height of 3-4 feet.
