Transitions Healthcare Gettysburg is seeking a dedicated and skilled LPN Assessment Coordinator to join our team.
**Primary Responsibilities:**
- Collaborate with the MDS Coordinator to develop and implement effective MDS completion strategies, adhering to facility procedures and state and federal regulations.
- Gather and compile necessary data for MDS completion.
- Identify significant changes in residents' conditions and monitor care plans for accuracy and completion.
- Attend Medicare meetings and participate in education sessions for nursing staff and interdisciplinary teams.
- Enter MDS information into the system and review messages from contracted pharmacies regarding medication prior authorization.
**Requirements:**
- Current licensure as a Practical Nurse or Registered Nurse in Pennsylvania.
- CPR/AED certification.
- Minimum of one year of nursing experience.
- Proficiency in MS Office Suite and Outlook.
