The schedule for this position is Wednesday and Friday 4p-12a and Saturday and Sunday 12p-12a (negotiable)

Responsibilities
- Patient Follow-Up: Return or make follow-up phone calls to patients as directed by the Triage RN.
- Assessment & Recommendations: Under the guidance of the Triage RN, make recommendations related to patient assessments, ensuring adherence to policies and procedures.
- Collaborate with Physicians: Communicate any changes in patient status or needs with attending physicians or TL on-call physicians.
- Patient & Caregiver Education: Instruct patients and caregivers on any changes to the patient’s Plan of Care.
- Coordinate Additional Care: Assign visits to RN, Social Worker, or Chaplain when necessary, in collaboration with the Triage RN.
- Licensure: Current LPN license in North Carolina (BSN preferred).
- Experience: Previous experience in hospice, home health, or palliative care is a plus.
- Transportation: Must have reliable transportation to visit patients in the service area.
- Compliance: Must pass all pre-employment and annual tests and meet state and federal guidelines.
