Hospice RN Case Manager, Full-time
Transitions LifeCare seeks to hire full-time Community-based Hospice RN Case Managers who embrace our mission of caring for individuals and families with compassion, support, and expertise as they navigate the end of life or life-changing illness.
This is an exciting time to join our team! Qualifying Experienced RNs with at least (1 years of experience) may be eligible for up to a $20,000 Sign-On* Bonus! This bonus is available for select positions with variable total amount dependent upon priority need, job territory, and other qualifying factors. *Details and Conditions apply.
The schedule for this position is Monday-Friday 8:00am-5:00pm; available one weekend shift per quarter; one major or two minor holidays per year.

- 6 weeks of paid time off (PTO) for 40-hour employees.
- Mileage reimbursement for travel to patient homes and facilities.

- Exhibit ability to make responsible choice and decisions that are in patients’ best interest
- Plan, provide, supervise and document safe, competent and compassionate hospice nursing care for patients
- Work collaboratively with the IDG Team and other internal/external partners in achieving patient care goals
- Use professional nursing judgement to individualize the plan of care based on assessment of the unique needs of each patient.
- Maintain comfort, privacy, and dignity of our patients
- Develop working knowledge of all federal, state and company regulations, policies, and procedures; ensure compliance
- Registered Nurse with current NC license; BSN preferred.
- Must have reliable transportation to travel to, enter and function in the homes/facility of any patient/family in the service area to assess and provide patient care.
- Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
- Hiring manager or Human Resources will contact you within 1-3 business days of your application submission
- Complete 1-2 interview(s) with the hiring manager via virtual video, or in-person
- Upon receiving the job offer, review job description, and complete new-hire tasks and paperwork (via UKG)
- Complete New-Hire Orientation and clinical skills training
- Receive continuous training from our team as you continue to provide exceptional care to our patients
