What are the responsibilities and job description for the Care Coordinator position at Transitions of Western Illinois?

Join our team as we GROW!!! We're expanding our services to high-risk children/families and are looking for Care Coordinators to provide high quality care coordination, crisis stabilization, community support, and discharge planning services to children who have mental illness. You must be able to form positive working relationships with family members and other service providers in order to best support the children in this program. Services will be provided in the clients' natural environment (home, community) within Adams and surrounding counties. Applicants must have:
- BA/BS degree in Psychology, Social Work, or other Human Services field
- Valid driver's license, reliable transportation, and a good driving record
- Ability to pass multiple background checks/drug test
- One year experience as a Mental Health Professional preferred
- Experience working with children/adolescents strongly preferred
Full-time position! Primarily daytime hours of work Monday - Friday with two evenings/week and flexibility to accommodate unplanned client/family needs with little notice plus occasional on-call coverage after normal business hours.
