Job Summary:
We are looking for a detail-oriented and compassionate LPN Care Coordinator to join our team in this new position! The LPN Care Coordinator will be responsible for providing high quality nursing, care coordination, and discharge planning services to adults and children who have mental illness. They will also coordinate and organize medical and mental health care services for high-need clients, sharing important information with the treatment team in order to promote a more safe and effective standard of care for those clients.
Work is performed in a fast-paced outpatient clinic and offsite in the clients' home/community locations.
Duties:
- organize and participate in medication clinics
- administer injections of medications as prescribed
- collect, store, and process laboratory specimens ordered by providers
- Assess client risks and social determinants of health , coordinate referrals to other providers, organize client care activities, and follow up on patient care
- Maintain timely and accurate client records
- Coordinate communication between clients, healthcare providers, and other staff members
- Provide transition and/or discharge planning based on client needs and continuity of care with other providers
Requirements:
- LPN licensure in Illinois (or ability to obtain within 30 days)
- Previous experience in a psychiatric or mental health facility is preferred
- Proficiency in using computer software such as Microsoft Office and an EMR
