• Qualifications :
• Master’s degree in behavioral health with active Kentucky License including CSW, LCSW, LPCA, LPCC or LPA and / or Certified by the state of Kentucky as a Certified Alcohol and Drug Counselor (CADC).
• Position Summary :

The Intake / Assessment Therapist completes initial clinical assessment and advocates for clients from crisis intervention through admission, collaborating with the medical team and Clinical Supervisor.

• Job Responsibilities :
• Complete clinical assessment for new intakes at the Residential Facility and determine appropriate level of care recommendations.
• Complete authorization for new admissions when needed including assisting in the peer to peer and appeals process.
• Fulfills productivity and documentation requirement on a weekly basis.
• Provides clinical coverage when needed, including but not limited to treatment planning, group and individual counseling.
• Participates in quality assurance related to the intake and assessment process.
• Demonstrates an independent work initiative, sound judgment, diplomacy, tact and professional demeanor.
• Excellent verbal, written and / or interpersonal communication skills.
• Provide quality care services to clients including clinical assessments, individualized treatment plans, individual therapy and group therapy.
• Participate in weekly multidisciplinary team meetings with support staff.
• Represent Transitions Inc. in a positive and professional manner in all community interactions.
• Provide high quality, compassionate guidance in both individual and group counseling sessions.
• Evaluate patient needs and determine if referrals to other programs or facilities are needed.
• Create and facilitate a discharge plan for each client on your caseload, including referring clients to a lower level of care and creating a safe housing plan.
• Collaborate with multidisciplinary team to determine clinical recommendations for each client’s length of stay and discharge plans
• Communicate and coordinate with referral sources, as necessary
• Provide crisis intervention to clients as needed.
• Act as a liaison between referral sources and clients.
• Performs other related duties as assigned.
