Traverse Health Clinic is actively seeking a Peer Recovery Navigator that functions as a key part of a team to meet the mission and objectives of our long-standing and engaged healthcare Clinic in Traverse City! Serves as a guide to initiate, achieve and sustain long-term recovery from addiction including medication assisted, faith based, 12 step and other pathways to recovery. This position is one part of an Integrated Physical Health and Substance Use Disorder treatment model, offering Medication Assisted Treatment (MAT).

We are not large, but have been a mighty Clinic in TC for over 45 years uniquely supporting both Primary Care and Behavioral Health services in one location.
Full-time, benefit eligible position with generous paid time off provisions and stable hours.
KEY FUNCTIONS
- Assists patients in setting and achieving personal goals.
- Coordinates and provides access to individual and family supports, including referral to community social supports.
- Utilizes knowledge of lived experience to assess patient needs, model healthy self-care strategies and help to reduce barriers to treatment.
- Identifies community resources (i.e. social services, workshops, etc) for patient to utilize to maximize wellness.
- Coordinates and provides access to chronic disease management including self-management support.
- Implements wellness and prevention initiatives.
- Facilitates health education and/or recovery-oriented groups.
- Provides education on health conditions and strategies to implement care plan goals including both clinical and non-clinical.
- Uses Recovery Coaching and the Recovery Pathways to maximize the opportunity to create recovery partnerships and a recovery environment.
- As part of the team assists, addresses and promotes wellness in all areas; physical, emotional, social, mental and spiritual.
- Through skill building interventions and other supportive interventions, assists patients in effectively utilizing a wide spectrum of resources, managing symptoms and developing daily living skills to gain greater independence, employment and other meaningful activities.
- Provides Screening, Brief Intervention and Referral to Treatment (SBIRT) for applicable patients to determine and identify individuals in need of treatment services, including MAT services.
- Works closely with the Substance Use Disorder Treatment team which includes primary care providers, nurse care manager, and behavioral health providers.
- Meets regularly and communicates with the care team to plan care and discuss cases, and exchanges appropriate information with team members in an informal manner as part of the daily routine of the clinic.
- Acts in a professional manner reflecting mission and philosophy of the Traverse Health Clinic.
QUALIFICATIONS
- High School Diploma or general education degree (GED).
- Certified Peer Recovery Coach can be State of Michigan, CCAR or MCBAP certified.
- Minimum of 2 continuous years in recovery from addictions with experience in navigating treatment services and/or prevention.
- Willingness to share his/her own story as a way to help others.
- Ability to adapt to change, motivate others and provide empathetic support.
- Knowledge of multiple recovery pathways. (MAT, AA, NA, etc)
- Ability to work a flexible schedule; excellent interpersonal skills; organizational skills; basic typing and computer skills.
- Ability to relate well with diverse populations of varying socio-economic backgrounds.
