COMMUNITY HEALTH RESPRESENTATIVE (CHR) - TRENTON COMMUNITY CLINIC JOB SUMMARY
Purpose
The Community Health Representative (CHR) Program provides quality care, health promotion and disease prevention services throughout the Trenton Indian Service Area to eligible program clients. CHR’s serve as a link between the clinical setting and the community to facilitate access to services and improve the quality of services in areas of general health care, dental care, gerontology care, and home health care. They assist by increasing health knowledge of patients and communities through a wide range of activities such as transportation to health visits, outreach, community education, informal support and advocacy. This is a regular, full-time role, Monday – Friday, 8:00 AM to 4:30 PM.
Responsibilities
- Promotes an organizational culture that embraces the values of respect, care, support, effective, communication, teamwork, collaboration, accountability, and responsibility; and works with all staff to create and maintain a workplace environment reflective of such values
- Provides health information, education, and instruction to clients to assist in the improvement or elimination of health concerns as it relates to general health care, dental care, gerontological health care, and home health care.
- Provides a variety of support services through the CHR and Title VI/Caregiver Assist Program, which furthers the services to our clients including care for elderly, disabled, homebound or with light duty household chores, preparing food, or assisting with personal care such as bathing or hair washing, home safety, etc.
- Take routine vital signs of patients, including temperature, pulse, respiration, blood pressure and pulse oximetry
- Provides care at the request of other health care professionals, clients or family members
- Identify and evaluate the needs of clients, develop, and implement intervention activities while assisting clients in accessing appropriate resources
- For coordinating services and/or resources with other health care professionals, clinic staff or other agencies to provide the highest quality of care to clients
- Provide transportation services to clients without other means of transportation for healthcare related needs, when necessary and available, such as conducting home visits, providing health education and maintaining health surveillance; to and from medical appointments; prescription delivery as needed and scheduled; coordinate other transportation needs as they arise in regard to caring for eligible service area members; deliver senior meals to clients who are homebound, etc.
- Additional travel may be required for other services, trainings, seminars, etc.
- Actively participate in the programs success by supporting defined goals and objectives of the program
- Participate in necessary and required trainings to improve skills to act as internal resource persons in the areas of care described above; additionally, each CHR must prove competency and genuine interest in the dedication to the CHR program and the health of all clients. Trainings will include basic, specialty and advanced trainings, as well as periodic refresher trainings to ensure skills remain functional
- Ensure compliance with program requirements and collaborate with appropriate company personnel as needed and required
- Maintain client electronic records and data collection systems vital to tracking and documenting the program, services, and activities of the CHR/Title VI programs
- Communicate relevant and necessary information to appropriate staff while adhering to confidentiality standards
- Participates in scheduled department meetings as required and requested
- Practices safe work habits in compliance with safety rules and regulations
- Utilizes resources appropriately and adheres to all guidelines, policies, and procedures
- Adheres to, and upholds HIPAA regulations, requirements, and trainings

Physical Demands
- Regularly required to stand, bend, talk, hear and lift up to 20 pounds
- Ability to withstand extended periods of travel in work provided vehicles, with occasional overnight travel
- Fine motor skills
- Visual acuity
- Involves direct and frequent contact with clients
Qualifications
- High school diploma or GED equivalent
- Previous experience in similar CHR or health care support role preferred
- Current CPR/First Aid Certification preferred
- Valid driver’s license with acceptable driving record
- Must be willing to travel as needed and requested
- Must be willing to meet all training requirements
- Ability to collect, summarize and report data coherently and logically
- Intermediate to advanced computer skills to sufficiently input and record client and program data; prepare reports as required and requested
- Demonstrate awareness, sensitivity, and appreciation of Native American culture, traditions, customs and needs
- Excellent interpersonal and communication skills and empathetic in client interactions
- High standards of ethics and confidentiality when handling sensitive information
- Ability to successfully pass all pre-employment requirements including a drug/alcohol test and background investigation
