What are the responsibilities and job description for the Community Outreach Specialist, ADRC (formerly known as ServiceLink) position at Tri-County Community Action Program, Inc?
* Leads outreach activities and participates as requested in inter-agency collaborations.
* Actively participate in community meetings, community collaborations and affiliations to advocate for ADRC (formerly known as ServiceLink) clients.
* Conducts home visits with clients as scheduled or requested.
* Provides support to clients and staff as identified for the program, help individuals access and make connections to services and supports, and conducts community outreach to identify supports, services, and connect or develop partnerships in support of clients and staff.
* Maintains all required trainings and certifications. These include, but are not limited to, SIRS, Medicare, Person Centered, Navigate.
* Assists with Medicare Open Enrollment during the Annual Open Enrollment Period.
An associate degree in Human services or a related field or experience may be considered.
Job Type: Full-time
Pay: $17.00 per hour
Expected hours: 30 – 35 per week
