What are the responsibilities and job description for the Office Intake Coordinator position at Tribute Home Health Care?
We are looking for an individual who has previous work experience in the home health field and possesses good communication, teamwork, and organization skills.
-Having computer knowledge for data entry and proficiency in English verbal and writing skills is also essential.
-Must have experience and be competent with Data Soft Logic database
-Contacting patients and providers to coordinate care and obtain documentation
-Check eligibility for direct and support services, obtain orders, and refer patients to needed services such as physical therapy, occupational therapy, speech therapy, social worker services, and mental health.
-Scheduling and coordinating with nursing staff
- Maintain patient documentation and confidentially track their information and schedules.
- Communicating with providers (doctors offices and hospitals)
- Communicating with patients for wellness follow-up checks
- Other assignments
Qualifications
- Experience and competency with Data Soft Logic database
- Proven experience in medical administrative support and office management
- Strong communication and organizational skills
- Proficient in data entry and maintaining medical records
- Knowledge of HIPAA regulations and some medical terminology
- Excellent customer service skills and experience handling multi-line phone systems
- Competence in Microsoft Office and other relevant software applications
