Qualification
- Advanced knowledge of Word, Excel and Power-point. Telephone etiquette Medical Terminology Knowledge of durable medical equipment and patient supplies.
- Minimum of three years’ experience, including two years of home healthcare experience in Quality Assurance
- Working knowledge of Medicare home health requirements
- Strong oral and written communication skills and interpersonal skills

Responsibilities
The Intake Coordinator will be responsible for the coordination of home care services to prospective clients and referrals
Handling all incoming referral calls and completing referral forms
Communicates with and documents all communications and authorizations with staffing coordinators and other necessary departments
Collaborates with discharging personnel, insurance, case managers to facilitate safe, early discharge
Maintains liaison relationship with hospital and insurance personnel, families, referrals, providing information and education on our organization’s services, coverage issues, and related areas of expertise
Responsible for transcribing detailed and accurate information in the computer system correctly to include but not limited to demographics, insurance information and prospective client information
Communicates with client and family regarding arrangements for initiation of care/services
Works closely with Staffing Coordinating department to ensure adequate care
Perform other duties assigned by supervisor
