JOB DESCRIPTION
In general, the Licensed Clinical Social Worker (LCSW) will be responsible for offering support services and providing behavioral health clinical case management to Soldiers in the designated area. Candidates for this position must be able to assist with potential behavioral health crises. The LCSW must maintain availability by phone and be available to travel throughout the State of Wyoming as this will be a necessary function to be assessed and completed as needed for this position. In addition, the candidate will be responsible for providing BH (behavioral health) recommendations, coordinating service member care, conducting evaluations and assessments for walk in Soldiers during all medical readiness events, referrals, self-referrals, and referrals for Chapter 3 that may require a temporary consolidate and maintain weekly metrics, perform initial and follow-up review to ensure quality of patient care, comply with and maintain access all regulations and guidance applicable to medical operations and programs of the Army National Guard.

POSITION TYPE & RATE INFORMATION

Salaried (Exempt) Annual Range $70,000.00- $85,000.00 USD (DOE.)
ASSOCIATED JOB DUTIES (Including but not limited to)
- Conduct evaluations and assessments for walk-in Soldiers during all medical readiness events, command-directed referrals, self-referrals, and referrals for (a) Chapter 3 that may require a temporary profile.
- Utilize the Behavioral Health Data Portal (BHDP) for assessments and evaluations.
- Consolidate and maintain weekly metrics (Appendix A) regarding Soldiers in the BH Program.
- Coordinate service member care including consultations for those initially using Military Treatment Facilities (MTF), Veterans Affairs (VA), TRICARE network providers, and/or non-network providers.
- Establish a list of available local BH resources within the state to coordinate care and ensure service members have alternative options available to receive medical care if not eligible for military health benefits.
- Provide at least three referral sources to SMs who are recommended treatment, before they leave the office.
- Ensures the SM understands their rights, benefits, privileges, and responsibilities IAW AR 40-501 Chapter 9 and 10.
- Coordinates with SM to ensure all medical readiness issues are properly addressed to minimize the amount of time in non-deployable status.
- Performs initial and follow-up reviews to ensure quality of patient care. Follow-up intervals will be based on the severity of each case.
- Record all correspondence with SM in BH Care in the corresponding military system (i.e. e-Case).
- Follow policies and procedures for reporting non-compliant Soldiers.
- Complete DA Form 3822 Mental Evaluation Form and scan it into HRR.
- Safeguard and maintain case folder for all active cases.
- Maintain medical records from civilian or VA Providers for service members they are case managing and request using form DD 2870, Authorization for Disclosure of Medical or Dental Information.
- Ensure completion of the required checklist and proper handling of all medical records (document management).
- Staff cases with other LCSWs and PHCs weekly and BHOs monthly
- Review E-case to ensure proper tracking and timeliness of all BH cases.
- Write temporary profiles as necessary utilizing e-Profile IAW 40-501.
- Additional BHCCM administrative duties as required or assigned.
QUALIFICATION REQUIREMENTS
- Required- LCSW with a valid License in good standing in the state of WY.
- Required- Intermediate to advanced (3-three years minimum experience) using MS office experience required.
- Preferred-CCM.
- Required- LCSW with a specialty in behavioral health, with a current state license (unrestricted).
- Excellent interpersonal communication skills
- Required- Advanced knowledge (minimum of 3-three years’ experience) using/ managing medical/clinical documentation related to functional impairment.
- Required- Minimum of 3-three years’ experience mentoring/advising claim managers on effective behavioral health clinical case management strategy.
- Preferred- Case management experience in behavioral health and disability/absence management.
- Preferred- Extensive experience with the Medical Disability Guidelines or Utilization Review Treatment Guidelines.
- Preferred- Credentials in related fields (e.g., CDMS, ADMS or CCM certification(s)).
