JOB DESCRIPTION
In general, the Lead Case Manager (LCM) will be responsible for offering support services and providing behavioral health clinical case management to Soldiers in the designated area. Candidates for this position must be able to assist with potential behavioral health crises. The LCM must maintain availability by phone and be available to travel throughout the State of Wyoming as this will be a necessary function to be assessed and completed as needed for this position. In addition, the candidate will be responsible for providing recommendations, coordinating service member care, conducting evaluations and assessments for walk in Soldiers during all medical readiness events, referrals, self-referrals, and referrals for Chapter 3 that may require a temporary consolidate and maintain weekly metrics, perform initial and follow-up review to ensure quality of patient care, comply with and maintain access all regulations and guidance applicable to medical operations and programs of the Army National Guard.

POSITION TYPE & RATE INFORMATION

Salaried (Exempt) Annual Range $60,000.00- $65,000.00, USD (DOE.)
ASSOCIATED JOB DUTIES (Including but not limited to)
  • In addition to the Care Coordinator/Case Manager responsibilities, the LEAD CASE MANAGER RESPONSIBILITIES include, but not limited to:
  • Works closely with all staff to ensure medical readiness
  • Works with Care Coordinators on all aspects of Case Management and ensures proper tracking.
  • Trains Care Coordinators and provides direction on required accesses and documentation. May delegate some training tasks to Care Coordinators.
  • Assist Care Coordinators with questions and provides support where needed
  • Works closely with senior staff on all aspects of administrative support, LOD tracking, MEB cases, cross-talk, etc…
  • Leads and overseas all Care Coordinators and keeps contractor informed on all aspects of Care Coordinator work performance and process.
  • Performs office management duties, works with POCs to ensure all Care Coordinators possess the proper tools to perform their job duties.
  • Assists contractor with tracking leave requests, LWOP, appointments, etc.
  • Leads weekly meetings with Care Coordinators and provides status updates, training and/or feedback
  • Provides local resources (as available) to Care Coordinators to ensure alternative options available to improve medical/dental readiness
  • Participates in community-based meetings or other events/workshops as per the DSS and approval of Contractor
  • Delegates/assigns cases to Care Coordinators and performs quality control on case reviews
  • Oversees and tracks all HIARNG members assigned in case management process and in the eCase module
  • Track 7281s
  • Maintains case load:
    • Manage treatment and care
    • Discuss treatment plan with provider
    • Obtain medical records
    • Upload medical records
    • Complete tracking reports
  • Completes monthly metrics report to contractor regarding all cases
  • Must have experience leading/managing a team with the ability to support the Case Management Team.
  • Must be able to handle high volume peak-times and multiple tasks with the ability to prioritize. Must work well with all colleagues and levels of management.
  • Must be RN, LVN or SW and possess active unencumbered license (RN, LVN, LCSW).
  • MUST have case management experience with Service Members and understand systems (eCase, MODS, MEDPROS, etc)

QUALIFICATION REQUIREMENTS
  • Required- A valid License (LCSW, RN, LVN) in good standing in the state of WY.
  • Required- Intermediate to advanced (3-three years minimum experience) using MS office experience required.
  • Preferred-LCSW RN.
  • Required- LCM with a experience in behavioral health, with a current state license (unrestricted).
  • Excellent interpersonal communication skills
  • Required- Advanced knowledge (minimum of 3-three years’ experience) using/ managing medical/clinical documentation related to functional impairment.
  • Required- Minimum of 3-three years’ experience mentoring/advising claim managers on effective behavioral health clinical case management strategy.
  • Preferred- Case management experience in behavioral health and disability/absence management.
  • Preferred- Extensive experience with the Medical Disability Guidelines or Utilization Review Treatment Guidelines.
  • Preferred- Credentials in related fields (e.g., CDMS, ADMS or CCM certification(s)).

    • Days- Monday- Friday (Must be available to travel to other locations within the State of Wyoming}

    • Required to travel to alternative work sites to attend/ participate in scheduled medical readiness events.
    • The candidate will be entitled to expenses reimbursed* (*=restrictions on reimbursement items may apply) by the Joint Travel Regulations/ JTR and reimbursed per the current GSA rate at the time of travel.
