Seeking full time Nurse Practitioner Manager for Population Health Medical Programs. The SNF medical program serve patients in local SNF facilities. The Primary Care at Home program provides home-based primary care. Community-based or acute care clinical provider and leadership experience required.
Job Overview:
This position is responsible for providing direct clinical care as a nurse practitioner, and for developing, implementing, and managing programs for patients in skilled nursing facilities, homes, and community settings to achieve population health goals of right care, right time, right setting, and right cost. Directly manages and leads NPs, RNs, SWs, Operations Coordinators, and other team members. Works in partnership with collaborating physician, population health team, primary care medical providers, specialists, referral partners, community agencies, and health plans. Ensures high reliability and excellent patient care outcomes to maintain health, treat illness, and prevent avoidable ED, hospital, or other healthcare utilization through process-driven, standard work. Establishes clear program budgets, goals and measures and manages outcomes through performance monitoring and improvement initiatives. Reports to System Executive Population Health Clinical Programs.
Job Requirements:
Master's Degree in Nursing
Graduate from Nurse Practitioner Program.
4-5 years experience, Clinal Nursing
Skilled nursing facility, home-based, clinic, community, or acute care setting.
3-4 years Management, Healthcare
Experienced managing and leading people, budgets, and programs in healthcare.
Advanced Practice Registered Nurse- Nurse Practitioner
Job Responsibilities:
Practices clinically and prescribes within full scope of APRN licensure in collaborative agreement with one or more physicians in a standard care arrangement. Follows evidence-based guidelines and best-known practices to ensure the right care at the right time in the right setting at the right cost. Provides goal-centered care to patients while supporting patients' families and caregivers. Collaborates with interdisciplinary teams across care continuum settings to ensure patient care is well-coordinated. Ensures comprehensive clinical documentation and coding. Facilitates safe and effective transitions of care.
Provides management and leadership to the development, implementation, and improvement of programs for patients receiving care in skilled nursing facilities, patient homes, and community settings. Creates budgets and manages revenue, expenses, and productivity to achieve budget targets. Stays abreast of emerging clinical models and practices. Viewed as subject matter expert resource in home, community, and post-acute programs.
Provides people management through hiring, managing, and leading NPs, RNs, SWs, Operations Coordinators, and other team members in accordance with TriHealth SERVE values and the TriHealth Way of Leading.
