- Assess, plan, and administer physical therapy treatments for patients in their homes.
- Develop personalized treatment goals based on patients' individual needs.
- Educate patients and caregivers on therapeutic exercises and safe mobility practices.
- Coordinate with other healthcare team members to ensure comprehensive patient care.
- Document treatments and progress accurately in accordance with regulatory and company standards.
- Current State of Michigan licensure as a Physical Therapist.
- Minimum of one year of experience in physical therapy, preferably in a home health or related setting.
- Strong communication and organizational skills.
- Ability to work independently and manage a caseload in a home setting.
