What are the responsibilities and job description for the Housing Case Manager position at Trinity Health Michigan?
Position Summary:
Under general supervision works with people living with HIV (PLWH) to assess eligibility for housing assistance, provides housing case management, conducts ongoing assessment and referral to appropriate support services, conducts home visits and housing inspections, engages landlords around working with clients who have complex needs and provides structured education to PLWH about housing skills in an individual and group setting and supports linkage and access to HIV care. Completes required program data entry and reporting. This position provides input in program specific policy and procedure using practice experience and supports completion of quarterly and annual reporting, ng annual budgets, adhering to funding requirements and allocations and educates colleagues about available housing resources.

What you will need:
EducationMinimum: High School Diploma
Preferred: Bachelor's Degree
Credentials/Licensure
Minimum: High School Diploma or equivalent, HIV Case Mgt. Certification within 1st year of hire.
Preferred: Bachelor's Degree
Related Experience
Minimum: Any combination of health/social services experience and/or education. One year of experience working in a community-based setting.
Preferred: One year Case Management or comparable work.
What you will do:
- Conducts housing, financial and social determinants of health assessment(s) and reassessments with patients at defined intervals.
Address referrals, triage client needs, supports client navigation to most appropriate housing services and maintains a waitlist when necessary.
Comprehensive, individualized /Service Plan, focused on removing barriers to HIV care through housing stability, developed collaboratively with the client within 30 days of assessment or referral for services.
All other duties as assigned.
