What are the responsibilities and job description for the Community Health Worker position at Trinity Health Of New England?
Position Purpose:
The Community Health Worker will lead outreach activities and coordinate to the efficient and effective implementation of Wise women outreach activities including developing partnerships, organizing events and assisting with follow-up services as needed.
What you will do:
Develops and implements an outreach plan for education and program activities including goals for the number of events; the venues and the numbers of people reached. These activities may include small group activities as well as more formal presentations to larger groups and screening within the community when feasible.
Provide health coaching and motivational support to participants on a routine basis to ensure active and engaged participation in their own health outcomes in the promotion of self-care skills and other follow-up care.
Proficient in MS Office Suite including Excel, Word and PowerPoint, and other appropriate software.
Works with Hospital clinics, emergency department and community programs to provide opportunities to educate women in the neighboring Hartford community about the importance and availability of Wisewomen preventative education and screenings.
Minimum Requirements:
High school diploma
Any combination of 3 years health/social services experience and/or education.
Some college, or Associate’s Degree preferred. Community Health Worker core competency training is a plus
Bilingual, English/Spanish a plus
