Targeted Case Management
Welcome to Trinity Prime Targeted Case Management. Our targeted case managers (TCM) are a very important part of our team. The role of a TCM is vital to client care. You will be responsible for assessing the needs of the client, assist the client with obtaining supplies and medical equipment, referrals, porvide suupport, and regularly monitor the client with home visits and phone calls.
Trinity Prime provides case management for two types of Arkansas Medicaid Clients:
* Medicaid Persoal Care Clients - must be over the age of years old and requires a signed prescription from the client's primary care provider.
* AR Choices Clients - Must be over the age of and approved by department of Human Services for the AR Choices program.
This candidate must be passionate, kind, serlf motivated, and a team player.
Travel involved with assistig those in need.
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
Expected hours: 40 per week
