Job Overview:
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The Hospice Nurse will provide end-of-life care and support to patients and their families during a challenging time. This role requires a high level of empathy, medical knowledge, and professionalism.
Responsibilities:
- Conduct initial and ongoing assessments of patients' physical, emotional, and spiritual needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Administer medications and treatments as prescribed by the physician
- Provide symptom management and pain control to ensure patient comfort
- Educate patients and families about the hospice philosophy and end-of-life care
- Provide emotional support and counseling to patients and their families
- Coordinate care with other members of the interdisciplinary team, including physicians, social workers, chaplains, and home health aides
- Document patient care services and progress in an accurate and timely manner
- Participate in on-call rotation as required
Experience:
- Previous experience in hospice or palliative care is preferred
- Proficiency in medical terminology and medical records management
- Strong patient care skills with a focus on comfort and dignity
- Knowledge of anatomy and physiology
- Understanding of HIPAA regulations
- Current nursing license (RN or LPN)
Qualifications:
- Current Registered Nurse (RN) license in the state of [Insert State]
