Job Description:
We are seeking a compassionate and dedicated Hospice Social Worker to join our interdisciplinary team. The Hospice Social Worker will provide psychosocial support to patients and their families, helping them navigate the emotional, practical, and financial challenges that arise during end-of-life care. The ideal candidate will have a strong understanding of hospice philosophy and a passion for supporting individuals and families during a challenging time.
Responsibilities:
- Conduct psychosocial assessments of patients and their families to identify emotional, social, and financial needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Provide supportive counseling and crisis intervention to patients and families
- Educate patients and families about the hospice philosophy and end-of-life care options
- Assist patients and families in accessing community resources and support services
- Advocate for patients and families to ensure their needs are met
- Assist with advance care planning, including completion of advance directives and living wills
- Provide bereavement support to families following the death of a loved one
- Document patient interactions and care plans in an accurate and timely manner
Qualifications:
- Master's degree in Social Work (MSW) from an accredited program
- Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of MA
- Minimum of 3 years of experience in a healthcare setting, preferably in hospice or palliative care
- Minimum of 1-year experience in Palliative or End of Life Care
- Strong assessment, counseling, and crisis intervention skills
- Knowledge of community resources and support services
- Compassionate and empathetic nature
- Ability to work independently and as part of a team
Preferred Qualifications:
- Certification in Hospice and Palliative Care Social Work (CHP-SW)
- Experience working with diverse populations, including geriatric and pediatric patients
- Bilingual proficiency (English/Spanish)
