- POSITION SUMMARY
- The Ambulatory Care Pharmacist (ACP) is a full-time employee who works collaboratively as part of an interdisciplinary team to help optimize medication use for outpatients of Trinity Health and assist with transitions of care for hospital discharges. This highly motivated and energetic pharmacist will perform patient interviews, review patient medical records, and execute comprehensive medication assessments to optimize medication management for patient care. The ACP will create and execute treatment plans through provider communication, patient education, and other care coordination modalities. The core objective of the ACP is to deliver high quality, evidence-based, cost-effective pharmaceutical care.
- MINIMUM QUALIFICATIONS AND REQUIREMENTS
- Licenses and Certifications Required
- Active Pharmacist License and must obtain a ND Registered Pharmacist license by the date of hire. Board Certification in Ambulatory Care is required within 18 months of hire.
- Educational Requirements
- Bachelors of Science Pharmacy degree or Doctorate of Pharmacy degree from an accredited school of pharmacy.
- Experience Requirements
- Minimum of two years, full time experience of relevant pharmacy practice experience.
- Successful completion of an American Society of Health-System Pharmacists accredited pharmacy residency preferred, but not required.
- Board Certification in Ambulatory Care is required within 18 months of hire.
- An interest in teaching pharmacy students and pharmacy residents is required.
- Ambulatory Care and Anticoagulation management service experience preferred, but not required.
- Special Skills or Training Requirements
- Comprehensive knowledge of common disease states and pathologies and ability to apply current pharmacologic and biopharmaceutical principles.
- Ability to comprehend patient medical management plans.
- Ability to extract pertinent information from the patient or other resources and relate the information to a medical management plan.
- Ability to serve as a preceptor for teaching/training purposes.
- Knowledge of laws, regulations and standards relative to Institutional Pharmacy practice and prescription dispensing.
- Understands hospital charging procedures.
- Knowledge of billing and coding rules/regulations for payors.
- Knowledge in pharmacy and computer applications.
- Ability to perform duties requiring independent judgment.
