What are the responsibilities and job description for the MSW Case Manager position at TriSource?
Overview
We are seeking a dedicated and compassionate Case Manager to provide direct social work services and case management to homeless individuals who are experiencing trauma.
Schedule: Monday-Friday 830am-430pm
Duties
- Conduct comprehensive assessments of patients to determine their needs and develop individualized care plans.
- Collaborate with healthcare providers, and other community agencies to provide support with benefit entitlement programs, employment and vocational rehabilitation services, mental health services, and other essential support services for shelter residents.
- Monitor and provide ongoing assessment to assist clients to become physically and mentally healthier, housed and in safe stable environments, and as independent as possible
- Ensure compliance with HIPAA regulations while managing sensitive patient information.
- Facilitate access to community resources and services that support patient recovery and rehabilitation.
- Advocate for patients within the managed care system to ensure they receive appropriate services.
- Document all case management activities accurately and timely
Experience
- Master's degree in Social Work
- Knowledge of resources for clients being served
- Experience working with at-risk populations and variety of mental health diagnosis
- Strong communication skills, both verbal and written, are required for effective patient interaction and documentation.
