True Believers LLC is an DHS certified outpatient mental health and substance abuse clinic. We believe that our consumers have the right to be treated with the utmost dignity and respect in all aspects of their lives. Our belief system is to meet people where they are and help them have optimum living and wellbeing. Our agency strives to meet our consumers hierarchy of needs by addressing their barriers. Our staff consist of licensed substance abuse professionals, licensed professional counselors, and master degree mental health providers with over 20 years of experience. True Believers LLC offers an array of services such as psychotherapy, psychoeducation, coping and/or grounding skills, meditation, medication management and limited case management services to all of our consumers. We also have staff that specifically specialize in gender dysphoria, transgender care, and transitioning. We work with clients to address ongoing concerns that they face using a variety of modalities and evidence-based practices. True Believers LLC is a non-judgmental space where we address issues facing our consumers.
The Behavioral Health Care Coordinator will facilitate services that ensure that members receive assessment services, service planning, service delivery, and supportive activities in an appropriate and timely manner. This includes ensuring that the service plan and service delivery for each member is coordinated, monitored, and designed to support the member in achieving the highest possible level of independent functioning. The Behavioral Health Care Coordinator will also assist members in self-advocacy and in obtaining other necessary services.
Responsibilities Include:
- Complete comprehensive mental health and substance use assessments and assessment summaries in a timely manner.
- Write DAP progress notes.
- Maintain accurate record keeping and submit all claims within two business days.
- Develop individualized recovery treatment plans.
- Order, review, and interpret medical records.
- Coordinate monthly recovery team meetings with consumers and team meetings.
- Work closely with medical providers to coordinate patient care.
- Work effectively with patients struggling with mental health issues.
- Complete required documentation of services.
- Collaborate with behavioral health and primary care providers and/or community services case managers for integration of care.
- Facilitate referrals to other needed services and/or levels of care, including coordination of care with all CCS providers.
- Ensure that client charts meet state and federal guidelines.
- Provide crisis prevention and intervention resources to consumers.
- Attend weekly individual and group supervision.
Position Qualifications:
- Master's degree in the behavioral health field and/or a job-related major
- State licensure (Licensed Professional Counselor, Licensed Clinical Social Worker) is preferred. Substance use credentials are also preferred.
- Valid driver’s license and successful completion of a driver’s background check for work related travel.
- Ability to pass the Wisconsin Caregiver Background Check screening process.
- Strong interpersonal and communication skills, both verbal and written.
- Excellent interviewing and assessment skills with demonstrated ability to listen attentively and empathetically to clients.
- Strong ability to translate assessment information into a treatment plan.
- Demonstrated success working with a diverse patient population and community.
- Demonstrated ability to manage stress and crisis situations.
- Knowledge of local, state, and federal community resources available to patients.
- Ability to work independently.
- Ability to maintain a high level of confidentiality.
