We are currently seeking a passionate and qualified Case Manager to join our team. This position is ideal for someone with a MHRT (Mental Health Rehabilitation Technician) certification and Targeted Case Management experience.
Position Summary:
The Mental Health Case Manager will work directly with individuals of all ages experiencing mental health challenges, providing assessments, individualized service planning, advocacy, and resource coordination. This role is integral to our mission of fostering connections and supporting recovery.
Key Responsibilities:
- Assessment and Planning:
- Conduct comprehensive assessments to identify client needs, strengths, and goals.
- Collaborate with clients to develop personalized service plans that promote independence and well-being.
- Service Coordination:
- Connect clients with community resources, including housing, employment, healthcare, and other social supports.
- Assist clients in maintaining benefits such as MaineCare, SNAP, SSI, and other programs.
- Advocacy and Support:
- Serve as an advocate to ensure clients receive the services they need.
- Offer emotional support to clients and their families to promote recovery and resilience.
- Documentation and Compliance:
- Maintain accurate and up-to-date client records in compliance with state and agency guidelines.
- Prepare and submit timely reports while ensuring confidentiality.
- Collaboration and Quality Improvement:
- Participate in regular supervision, team meetings, and professional development opportunities.
- Support quality assurance efforts by providing feedback and adhering to best practices.
Qualifications:
- Education: Bachelor’s degree in social work, psychology, counseling, or a related field.
- Certification: MHRT/C certification or eligibility to obtain it within 6 months of hire.
- Experience: Experience in case management, social work, or mental health services preferred.
- Skills:
- Strong interpersonal and organizational skills.
- Proficiency in documentation and electronic record-keeping systems.
- Knowledge of Maine’s mental health system and community resources.
- Requirements:
- Valid driver’s license and reliable transportation.
- Ability to pass a background check.
