JOB SUMMARY:
The Community Access Coordinator acts as a community liaison for current and prospective patients, connecting them with viable services offered by True Health and throughout the community.

The Community Access Coordinator acts as a community liaison for current and prospective patients, connecting them with viable services offered by True Health and throughout the community.
PRIMARY FUNCTIONS:
- Facilitate patient access to social programs including Medicaid, Medicare, Social Security, Disability, Supplemental Nutrition Assistance Program (SNAP), Florida KidCare and Temporary Cash Assistance while protecting the fiscal interest of True Health.
- Assist patients with each step of the marketplace enrollment efforts which includes initial application, determination of advanced premium tax credits (APTCs), medical and dental plan selections, and any other post-application service needed for the individual.
- Connect patients with True Health services by scheduling appointments for any service offered by the organization.
- Navigate patients through various social program options through one-on-one counseling.
- Assess the patient’s needs and make referrals to appropriate community resources.
- Track and report patient's progress in applying for assistance.
- Advocate on the patient's behalf to help resolve problems with resources.
- Following up with individuals to ensure their application receives a final determination or they receive the needed community assistance.
- Participate in community events geared at connecting prospective patients into True Health and other community resources. This may include after-hours and/or weekend events.
- Attend basic training provided by the Center for Medicare and Medicaid Services, the Florida Healthy Kids Corporation, and the Department of Children and Families.
- Other responsibilities as assigned
- High School Diploma or GED certificate
- Some college preferred
- Preferred - one or more years of experience as front office staff in a healthcare setting and customer service.
