Position Available: RN On Call
Job Duties and Responsibilities:
- Provide prompt and compassionate nursing care to patients in their homes during on-call hours, including evenings, nights, weekends, and holidays.
- Respond to urgent patient calls and triage situations to determine the appropriate level of care.
- Conduct thorough assessments of patients' conditions during on-call visits and implement necessary interventions.
- Administer medications, treatments, and procedures as needed, ensuring patient safety and comfort.
- Communicate with primary care physicians, hospice medical directors, and other healthcare providers to coordinate and update care plans based on patient needs.
- Offer emotional support and education to patients and their families, addressing concerns and providing guidance on symptom management.
- Document all patient interactions, assessments, and interventions accurately and timely in the electronic health record system.
- Collaborate with the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
- Participate in regular on-call rotations as part of the nursing team.

Qualifications:
- Registered Nurse (RN) license in good standing.
- Previous experience in home health care or hospice care is preferred.
- Strong clinical skills and knowledge of evidence-based practices in nursing.
- Excellent communication and interpersonal skills.
- Compassionate and empathetic attitude towards patients and their families.
- Ability to work independently and as part of a collaborative team.
- Flexibility to respond to patient needs during on-call hours.
