Position Available: RN On Call

Job Duties and Responsibilities:

• Provide prompt and compassionate nursing care to patients in their homes during on-call hours, including evenings, nights, weekends, and holidays.
• Respond to urgent patient calls and triage situations to determine the appropriate level of care.
• Conduct thorough assessments of patients' conditions during on-call visits and implement necessary interventions.
• Administer medications, treatments, and procedures as needed, ensuring patient safety and comfort.
• Communicate with primary care physicians, hospice medical directors, and other healthcare providers to coordinate and update care plans based on patient needs.
• Offer emotional support and education to patients and their families, addressing concerns and providing guidance on symptom management.
• Document all patient interactions, assessments, and interventions accurately and timely in the electronic health record system.
• Collaborate with the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
• Participate in regular on-call rotations as part of the nursing team.

Qualifications:

• Registered Nurse (RN) license in good standing.
• Previous experience in home health care or hospice care is preferred.
• Strong clinical skills and knowledge of evidence-based practices in nursing.
• Excellent communication and interpersonal skills.
• Compassionate and empathetic attitude towards patients and their families.
• Ability to work independently and as part of a collaborative team.
• Flexibility to respond to patient needs during on-call hours.
