Job Summary
We are seeking a highly skilled Population Health Strategist to join our team. In this role, you will lead the development and implementation of programs aimed at improving health outcomes for our patients. You will also be responsible for driving quality improvement initiatives and fostering a culture of continuous learning within our organization.
Key Responsibilities
- Develop and implement population health strategies that align with organizational goals and priorities.
- Lead quality improvement initiatives to enhance care delivery and improve patient outcomes.
- Foster a culture of continuous quality improvement and patient-centered care within our organization.
- Collaborate with clinical and operational leaders to ensure initiatives are integrated into workflows and care delivery.
Requirements
- Bachelor's degree in nursing from an accredited college plus a minimum of 5 years' clinical nursing experience.
- Previous experience leading quality improvement and/or change management processes and implementation cycles.
- Proven ability to lead multidisciplinary teams, including clinical, operational, and administrative staff.
- Registered Nurse (RN) licensure in California.
